Volunteer Service Awards
The Ontario Volunteer Service Awards recognize individual volunteers for continuous years of commitment and dedicated service to an organization.
Adults are recognized for five, 10, 15, 20, 25, 30+, 40+, 50+ and 60+ years of continuous service. Youth (under the age of 24) are recognized for two or more years of continuous service.
The awards are presented at special ceremonies held across Ontario where recipients are presented with a stylized trillium service pin acknowledging their years of service. Recipients also receive a personalized certificate.

Submit a Nomination
Nomination forms are available at the Administration Centre in Alliston, Joint Operations Centre in Beeton, and on the Ministry of Citizenship and Immigration website.
Nomination forms will be received until 4:30 p.m. on the third Friday in December of each year and should be addressed to:
